【共同声明】 グリーン保健医療システムの構築に向けた大きな一歩：世界保健機関（WHO）執行理事会における日本政府代表団による「気候変動と健康に関する 変革的行動のためのアライアンス（ATACH）」への正式な関心表明を支持

特定非営利活動法人日本医療政策機構（HGPI: Health and Global Policy Institute）（事務局：東京都千代田区、代表理事：黒川清）は共同声明「グリーン保健医療システムの構築に向けた大きな一歩」を公表しました。

日本医療政策機構のプラネタリーヘルスプロジェクトは、気候変動を含むプラネタリーヘルス領域の課題に対応する重要性が、年々高まっている現代社会において、2024年1月27日にジュネーブで開催された第154回世界保健機関（WHO: World Health Organization）執行理事会において日本政府代表団による「気候変動と健康に関する変革的行動のためのアライアンス（ATACH: Alliance for Transformative Action on Climate and Health）」に対する正式な関心が示されたことを高く評価し、全面的に支持します。

2023年12月に開催された国連気候変動枠組み条約（UNFCCC: United Nations Framework Convention on Climate Change）による第28回締約国会議（COP28）では、歴史上初めての健康の日（Health Day）が12月3日に開催され、気候変動が健康に与える影響について深刻な懸念が表明されました。また、同会議の気候・保健大臣会合では、「COP28 UAE 気候・健康宣言（COP28 UAE Declaration on Climate and Health）」が、日本を含む143カ国による署名のもとで採択されました。この宣言文は、国際社会における健康と気候変動の相互作用への認識を深め、国際的な協力を促進するものです。

当機構のプラネタリーヘルスプロジェクトにおいては、マルチステークホルダーにより日本国内外の医療政策課題の解決に向けた取り組みを続けております。2022年12月には、主要国首脳会議（G7）広島サミットのホスト国である日本政府に対して、ATACHへのコミットメントを求める「要望書：G7広島サミットおよび関係閣僚会合に向けて 国際社会と歩調を合わせた気候変動・プラネタリーヘルス対策の推進」を公表しました。ATACHは、気候変動に強靱で持続可能な保健健康システムの構築を目指し、各国の協力を促進する重要なイニシアティブです。

この度のWHO執行理事会における議論は、ATACHだけでなく、環境汚染など広範なテーマを包含しており、各国の足並み統一に向けた努力が重要であることを示しています。そのため、2024年5月27日から6月1日に予定されている第77回世界保健総会（WHA: World Health Assembly）に向け、気候変動以外にも、プラスチックなどによる環境汚染や生物多様性の喪失などが健康に与える影響について、どのような主体がどのように取り組みを牽引していけるのか注目が集まっています。

今後、日本政府はATACHへの参加プロセスを通じて、「気候に強靱な保健医療システムの構築」「低炭素持続可能な保健医療システムの推進」「サプライチェーンの環境影響の管理」「資金調達の促進」「気候行動と栄養の統合」などが求められることになります。プラネタリーヘルスの観点から持続可能な社会の実現に向けた新たな社会システムを設計することが喫緊の課題であり、日本政府の取り組みはこの分野における変革のドライバーとなることでしょう。また、国内の産業界では、日本製薬工業協会（JPMA: Japan Pharmaceutical Manufacturers Association）も「COP28「気候変動と健康に関する宣言」に対する革新的医薬品産業の声明」公表し、ATACHを歓迎しています。我々は、日本政府がこういった産業界などとイニシアティブを取り、国内でプラネタリーヘルス領域の課題に対するサイロを乗り越えた連携・協力の実現に向けた動きが進展することに大きな期待を寄せています。

我々は、日本国内だけでなく、世界に向けても有効な医療政策の選択肢を提示し、地球規模の健康・医療課題の解決に向けて、引き続きそれぞれの立場を超えて共に活動していきます。今後進められる国内のステークホルダーの理解促進に対して、当機構のプラネタリーヘルスプロジェクトとしても、可能な限り支援を実施してまいります。

A Significant Step Towards the Building a Green Healthcare System: Support for the Formal Expression of Interest by the Japanese Government Delegation to the ATACH at the Executive Board Meeting of the WHO

Health and Global Policy Institute's (HGPI) Planetary Health Project acknowledges and fully supports the Japanese government delegation's formal expression of interest in the Alliance for Transformative Action on Climate and Health (ATACH) at the 154^th World Health Organization (WHO) Executive Board in Geneva on January 27, 2024. This acknowledgement comes in light of the growing importance of addressing planetary health challenges, including climate change, in today's society.

The 28th Conference of the Parties (COP28) to the United Nations Framework Convention on Climate Change (UNFCCC), to be held in December 2023, held its first-ever Health Day on December 3, expressing serious concern about the impact of climate change on health. In addition, the "COP28 UAE Declaration on climate and health" was signed by 143 countries, including Japan, at the COP28 Climate and Health Ministerial Meeting. This declaration aims to raise global awareness of the interaction between health and climate change and to promote international cooperation.

HGPI's Planetary Health Project continues to address health policy challenges, both domestically and internationally, led by multiple stakeholders. In December 2022, we presented a "Request for the G7 Hiroshima Summit and Related Ministerial Meetings: Promoting Measures for Climate Change and Planetary Health That Keep Pace With the International Community" to the Japanese government, host of the G7 Hiroshima Summit, urging commitment to ATACH. ATACH is an important initiative to promote collaboration among countries to build climate-resilient and sustainable health systems.

Discussions at the recent WHO Executive Board covered a wide range of topics, including not only ATACH but also environmental pollution, demonstrating the importance of unified efforts among countries to align their efforts. Therefore, at the upcoming 77th World Health Assembly (WHA), to be held from 27 May to 1 June 2024, there is great interest in understanding how different entities will lead efforts to address the health impacts of environmental pollution, such as plastic waste, and biodiversity loss, in addition to climate change.

Moving forward, the Japanese government will be asked to commit to "Climate Resilient Health Systems," "Low Carbon Sustainable Health Systems," "Supply Chains," "Financing," and "Climate Action and Nutrition," through the ATACH engagement process. Addressing the urgent need to design new social systems for achieving a sustainable society from a planetary health perspective, the initiatives of the Japanese government are anticipated to be a driving force for change in this domain. Additionally, the Japan Pharmaceutical Manufacturers Association (JPMA) has also welcomed ATACH by announcing the statement "Innovative pharmaceutical industry statement on COP28 Declaration on Climate and Health," highlighting the industry's support. We have high expectations that the Japanese government will lead, in collaboration with industries such as JPMA, to overcome silos and realize partnerships and collaborations addressing planetary health challenges within Japan.

We will continue to present effective health policy options not only within Japan but also globally. We are committed to working together beyond individual perspectives to address global health and medical challenges on a planetary scale. HGPI's Planetary Health Project will strive to provide as much support as possible to promote understanding among domestic stakeholders in the future.
